We're a small outfit, just four guys doing mostly pressure vessel work in refineries. After a close call with a hot work permit mix-up about two years back, I got real strict about the morning huddle and double-checking isolations. The counter on our shop whiteboard hit 500 this morning. It matters because it proves the grind of daily safety talks and checking each other's gear actually adds up.
