Serious question, a guy on my shift said my handoff notes were useless and he was right
I've been giving report at 7am the same way for like 4 years, just reading off vitals and what the doctor ordered. Last Tuesday a nurse named Dave pulled me aside after handoff and said "I can't tell what's actually wrong with these people from your notes." At first I was annoyed, but then I read one back and he was right, it was all numbers and no story. So now I lead with the one line about what could go wrong in the next 12 hours and work backward from there. Has anyone else gotten feedback like this and actually changed how they write stuff up?
